Thank you for your interest in establishing yourself as a new patient in our clinic! We have established a standard process to make this flow smoothly, so please read the following carefully.
**Please note that we are suspending new patient scheduling in July to allow for a staffing transition. We anticipate being able to resume in August, but with very limited openings while we continue to adjust. **
- Adults must call and schedule themselves, unless they are disabled or cognitively impaired, in which case their guardian or caregiver may call on their behalf.
- New patient scheduling days are currently the first Thursday morning of each month. They fill up very fast! Upcoming new patient scheduling days: NONE in July due to a staffing transition, August 6th, September 3rd. Please monitor this page for updates to the date.
- Scheduling is done on a rolling basis for the third month out. For example, calling in January means you will be scheduled in March.
- There are a limited number of slots available for our two providers who are accepting new patients, Christina Maruyama and Candice Brunger. Our other providers are seeing established patients only.
- Appointments are on a first come/first served basis in the order that calls come in. We experience a very high volume of calls on scheduling day, so please be patient with us. To eliminate confusion about placement in the scheduling queue, we will not accept new appointment requests via text or voicemail, only direct conversation with our staff. We increase staffing on our scheduling day to facilitate this.
- If our main phone line, 801-252-6116, does not direct you to speak to one of our staff members, call our alternate number, 385-500-4796.
- Once you are scheduled, you will receive our intake forms via email and text. Please fill those out right away. If you have an urgent need and wish to be added to the cancellation list, we can do so as soon as those forms are completed.
